We offer home visits, virtual meetings, telephone support calls and much more to clients over the age of 65 who need support with coordinating their health and social care services. This includes a complete biopsychosocial assessment in their own home. Whatever your health and social goals, we have the expertise to help you achieve them whilst sourcing local resources for you.

Golden community provides a Registered Nurse who will visit you in the comfort of your own home to complete a biopsychosocial assessment and together help co-ordinate your health and social care that fits your lifestyle and goals. Whether you're looking for a personal assistant approach to coordinate existing complex health and social issues, or just want advice and help navigating around the Age well Government initiative for Ageing Well and supporting people living with frailty, our nurses will guide and support you to achieve your optimal health and wellbeing. Lorraine our Registered Nurse Manager is trained to complete health and wellbeing assessments including Falls risks, Diabetes Risk, Dementia screening, Frailty, MUST and much more. Lorraine is highly experienced in frailty and dementia. Lorraine will work with your individual goals to help you to identify any early signs of dementia , isolation and loneliness. Together we can plan to improve your overall health and well-being and support you to access local services. NHS England states by maintaining our health, fitness and resilience throughout life. We can significantly reduce the risk of diseases which are more likely to develop as we age. Once we develop diseases and disabilities during our life course, we can develop interventions which reduce the impact of those conditions on our lives and the services we use.
Golden community will invite you to join health talks online. This is an open forum with regular guest speakers to discuss health topics and local services. If there are any health topics you would like more information or be included please send an email to Lorraine and she will happily organise this. As part of the membership and Golden community mission to reduce loneliness and isolation, you will be invited to the facebook group and encouraged to take part in the weekly challenges and updates.
Golden community is available for telephone calls or virtual calls, it is always better to see someone face to face but sometimes a virtual call as a friendly face and catch up is a good start to the day. The virtual drop in clinic is an open clinic for members to drop in and catch up with each other daily. It is good for our mental health and wellbeing to check in and support each other, families are welcome too.

As part of Lorraine's assessment she will be able to look at the home environment, including safety and safeguarding concerns and direct you to your local support services. Together we can look at ways to support and coordinate community services in your area. Family and friends are everyone's priority but if you are feeling abandoned, isolated or lonely, Lorraine can work with you to build on future relationships and move forward with your personalised goals.
Golden Community understands and can support you through Advanced Care planning. This can be a difficult subject to broach with loved ones. Lorraine is highly experienced in dealing with difficult discussions regarding DNACPR/RESPECT forms and Lasting power of Attorney. Planning for future care is an empowering act that allows people to feel confident their wishes will be considered if they are ever unable to fully participate in decision making. By having good quality conversations about future treatment, people will have a greater sense of control over their ability to live well and die well.
On initial assessment and monthly reviews, we will look at using recommended tools to assess your general health status, functional independence/performance, cognition and social support. All assessments and health needs will be sent to your own GP for their records on your request. An individualised action plan to maintain optimum level of health and wellbeing will be completed together.
Frailty is now being considered a long term condition. By using the Rockwood assessment tool, Lorraine can assess and identify any needs that may be addressed to maintain or improve your frailty score. Using a Multidisciplinary approach we may involve local private physiotherapists and nutritionists to reach and maintain your optimum self.
If you're living with a chronic disease, we can help you manage your symptoms and improve your quality of life. Our team of healthcare professionals will work with you and coordinate your care with your GP and other services.
As part of your initial assessment, Lorraine will offer a memory screening test. Lorraine will support you through the process to contact your GP for further tests and referral to the memory clinic if required There are also local private companies who offer a full dementia screening and diagnosis if you are worried or showing the early signs of Dementia. Early diagnosis of dementia is key in tackling and treating the progress of the disease.
Lorraine has always had a passion in falls prevention. Falls in the elderly can lead to hospital admissions and even death. Lorraine will complete a falls risk assessment and discuss recommended ways to stay independent and safe. Together we can look at ways to minimise the risk of falls in the home and source local resources such as balance and exercise classes.
