Registered Nurses

Job Summary

We are seeking a compassionate and dedicated Registered Nurse to join our growing company Golden Community. The Registered Nurse (RN) will provide comprehensive nursing care to patients in our newly developed 6 bed Nursing home for complex chronic diseases, dementia, palliative and End of Life residents. The RN will be responsible for assessing patient needs and delivering high standards of personal care, developing care plans to support their personalised support plans for health, social and wellbeing, administering treatments and medications, and collaborating with other healthcare professionals to ensure high-quality patient care at all times. This is a unique opportunity to work in a small nursing home in a quaint village, with beautiful surroundings, ensuring residents receive the nursing care and support they deserve. The Nursing home will also provide day care, activities and wellbeing, such as regular health promotion and screenings, encouraging the community to be part of the Nursing support network.

Key Responsibilities:

· Patient Assessment and Care Planning:

o Conduct thorough assessments of patients' health conditions.

o Develop and implement individualised care plans based on patient needs.

o Monitor and evaluate patient progress, adjusting care plans as necessary.

· Direct Patient Care:

o Administer medications and treatments as prescribed by healthcare professions.

o Provide wound care, catheter care, venepunture and other nursing procedures.

o Assist with activities of daily living (ADLs) and provide support to patients and their families.

· Coordination and Collaboration:

o Coordinate with other healthcare professionals, including GPs, therapists, and social workers.

o Communicate effectively with patients, families, and caregivers to ensure continuity of care.

o Participate in multidisciplinary team meetings to discuss patient care plans.

· Documentation and Reporting:

o Maintain accurate and up-to-date patient records.

o Document all nursing interventions and patient responses.

o Prepare and submit reports as required by regulatory bodies and the healthcare facility.

· Education and Support:

o Educate patients and their families about health conditions, treatments, and preventive care.

o Provide emotional support and counseling to patients and their families.

o Train and supervise care aides and other support staff.

· Compliance and Safety:

o Ensure compliance with healthcare regulations and standards, including CQC, local and national standards.

o Follow infection control protocols and maintain a safe environment for patients and staff.

o Participate in quality improvement initiatives and continuing education programs.

Qualifications:

· Education:

o Registered Nurse with the NMC.

o Evidence of revalidation and CPD.

· Experience:

o Minimum of 1-2 years of nursing experience, preferably within a care home or community care settings.

o Experience with electronic health records (EHR) systems is a plus.

· Skills:

o Strong clinical and critical thinking skills.

o Excellent communication and interpersonal skills.

o Ability to work independently and as part of a team.

o Proficiency in using medical equipment and technology.

· Other Requirements:

o Valid driver's license and reliable transportation for community visits.

o Current enhanced DBS.

Working Conditions:

· The RN will work in various settings, including our care homes and patients' own homes.

· The role may require travel within the community and flexible working hours, including evenings and weekends.

Activity Co-ordinator/ Wellbeing Support worker

We are seeking a compassionate and dedicated Activities Coordinator/ Wellbeing support worker to join our team in providing enriching experiences for residents in our nursing home and community patients. The ideal candidate will have a strong background in senior care, particularly with dementia care, and possess excellent communication skills to engage with residents, families, and staff. As an Activity Coordinator/ Wellbeing support Worker you will be expected to assistance individuals with physical or mental health challenges, disabilities, or other health-related issues. The role involves offering emotional support, practical assistance, and advocating for the needs of clients to improve their quality of life. This role is pivotal in enhancing the quality of life for our residents through well-planned activities that promote social interaction, cognitive stimulation, and overall well-being.

Key Responsibilities:

· Emotional Support: Provide emotional support and practical assistance to clients.

· Daily Tasks: Help clients with daily tasks such as personal hygiene, preparing meals, and housekeeping.

· Mobility Assistance: Assist clients with physical mobility, including helping them walk or transferring them from a wheelchair.

· Appointments: Accompany clients to appointments and social events.

· Health Monitoring: Monitor the well-being of clients and report any health or wellbeing concerns to relevant health professionals.

· Medication Management: Assist with medication management, ensuring clients take their prescribed doses on time.

· Care Plans: Develop and implement individual care plans in conjunction with healthcare professionals to ensure their individual interests and hobbies are included in their support plan.

· Record Keeping: Create and maintain accurate records of clients’ progress and conditions.

· Social Activities: Encourage clients to participate in social activities and engage in the community.

· Event Planning: Organise and plan various activities and events, ensuring they align with organisational goals and objectives.

· Participant Engagement: Engage with residents and community patients to understand their interests, needs, and preferences to tailor activities accordingly.

· Logistics Management: Coordinate logistics including venue bookings, equipment, supplies, and transportation necessary for events and activities.

· Communication: Maintain effective communication with participants, colleagues, and stakeholders regarding upcoming events, schedules, and any changes or updates.

· Budget Management: Assist in managing budgets for activities and events, ensuring expenses are within allocated limits.

· Evaluation and Feedback: Collect feedback from participants and stakeholders to evaluate the success of activities and identify areas for improvement.

· Documentation: Maintain accurate records, reports, and documentation related to activities, attendance, and feedback.

Requirements

  • Previous experience in a similar role within a nursing home or senior care setting is highly desirable.
  • Knowledge of dementia care practices is essential.
  • Strong communication skills with the ability to engage effectively with seniors.
  • A creative mindset with the ability to design engaging activities that cater to diverse interests.
  • Compassionate nature with a genuine passion for improving the lives of seniors through meaningful interactions.
  • Ability to work collaboratively as part of a multidisciplinary team while also being self-motivated.
  • Flexibility to adapt programmes based on resident feedback and changing needs.

Support Worker

Responsibilities

  • Provide personal care and support to clients in assisted living or home care settings.
  • Assist with daily living activities, including bathing, dressing, and meal preparation.
  • Monitor clients' health and report any changes or concerns to the supervising nurse or healthcare professional.
  • Maintain a safe and clean environment for clients, adhering to health and safety regulations.
  • Engage clients in social activities to promote emotional well-being and companionship.
  • Drive clients to appointments or social outings as needed.

Requirements

  • Previous experience in patient care, or a similar role is preferred.
  • Training in Care certificate or NVQ level 2 preferred but training will be provided.
  • Strong interpersonal skills with the ability to build rapport with clients.
  • A valid driving licence is preferred for transporting clients.
  • A caring attitude with a genuine desire to help others.
