Proactive Community Nurse Specialist

Lorraine our Proactive Community Nurse Specialist, is a Registered Nurse with over 30 years experience in elderly care within care homes and the community. She has worked for both the private and NHS sector building on her skills and knowledge. Lorraine's passion remains in the community, visiting elderly patients, with complex issues, specialising in Dementia, Frailty, Palliative and End of Life, however is a firm believer in assessing healthy clients risks and building proactive action plans to manage the risks and prevent hospital admissions. 

Lorraine has a friendly proactive approach to put you at ease to share your recent challenges with coordinating your health and social care. Lorraine recognises the proactive service in the community is invaluable as many patients when they need to access health or social care have no idea what is available and where to turn. Lorraine is expert in providing a non judgemental attitude to ensure everyone receives the best available services in their area.

Lorraine encourages spouses, family and friends to be involved as it is a vital part of your wellbeing. She is happy to arrange meetings either face to face or virtually. Over the years virtual meetings have been a vital part of bringing people together especially during COVID. Virtual meetings work well when clients request their family members who live miles away to join in planning their future care and empowers the client and their family to be part of their future journey.
