Lorraine Lewis is the owner of Golden Community and is a Registered Nurse with over 30 years of experience in the community supporting care homes and house bound patients.

Specialising in Frailty, Dementia, Palliative and End of life coordination, Admission Avoidance and discharge reviews. Lorraine can provide expert advice on all your health and social needs to remain safe in your own home.

At Golden Community, we believe in a holistic approach to community care, recognising that true well-being encompasses more than just physical health. Our team of experienced trained Healthcare Professionals, Personal Assistants and Activity Co-ordinators are committed to creating a supportive and nurturing home environment where everyone can thrive.

Our Holistic Support Services include:

Wraparound support including: Cleaning, administration, pet care, shopping and meal preparation, travel assistance, prescription pick up, companionship and full befriending service.
Health and Wellness Programs: We offer a monthly maintenance services, including weekly Nurse and/or Support visits to suit your individual needs. With Advice and guidance on hand between visits via communication of your choice and ability.
In-Home Support: Delivering personalised support to individuals in their homes, including and not limited to Dementia and Palliative sitting service, activities and daily diary planning, tackling isolation and improving general wellbeing.
Virtual Nurse Support: For all your health and social co-ordination advice, our nurses can provide a virtual or telephone call to discuss referrals and resources in the community available, helping with the lengthy referrals and paperwork.
Wellbeing Visits: We provide on off or weekly/monthly wellbeing check visits either by a Registered Nurse or Personal Assistant, whether you are away on holiday or do not live nearby, book one of our trained team members to visit your loved one to check they are managing, e.g.: heating is working , food in fridge is in date, clean clothes etc.
End of Life Services specialising in: Coordinating end of life care and a nurse/carer sitter service. The nurse can double up with an existing care package and offer support for palliative patients, symptom control and advice, completing referrals and fast track continuing health care, liaise with professionals, discuss RESPECT and complete advanced care plans.
