Holistic Community Services

Golden Community provides nurse-led, holistic health, wellbeing and community support for adults aged 18+, helping people remain independent, connected and well in their own homes and communities.

We know that a person's needs rarely fit neatly into one box. Physical health, emotional wellbeing, independence, social connection, nutrition, mobility and confidence can all affect one another. That's why our multidisciplinary team of registered nurses, support workers and activity coordinators works together around the whole person.

Our support can range from companionship, shopping, meal preparation, meaningful activities and getting out into the community, through to proactive nurse-led wellbeing reviews, health monitoring and support for people living with frailty, dementia or long-term conditions.

We focus on prevention and early recognition – noticing small changes, responding appropriately and working alongside families and existing health and social care professionals when additional support is needed.

Whether someone needs a little support to stay connected or more regular input to remain safely independent, our aim is the same: to help people live well, maintain choice and continue doing the things that matter to them.

One of the things that makes Golden Community different is the involvement of our registered nursing team.

Where appropriate, our nurses can provide proactive wellbeing reviews and monitoring alongside the individual's usual NHS healthcare.

This can include monitoring areas such as blood pressure, weight, nutrition, hydration, frailty and general wellbeing, identifying changes and escalating concerns to the appropriate healthcare professional when needed.

We don't replace your GP, District Nursing Team or other NHS services. We work alongside them and help people and families navigate services when additional support is required.

Loneliness and social isolation are not simply social problems. They can affect confidence, independence, mental wellbeing and physical health.

That is why meaningful activity and social connection form an important part of our approach.

Our Activity Coordinators and wider team help people reconnect with interests, hobbies and their local community – whether that's going out for coffee, gardening, shopping, attending a group, taking a walk, enjoying music or simply having something meaningful to look forward to.

It's not about filling time. It's about maintaining purpose, identity, relationships and enjoyment of life.

Dementia, Frailty & Changing Needs

We support individuals and families living with dementia, frailty and long-term health conditions.

Our multidisciplinary approach means that changes noticed during an activity or support visit can be shared appropriately with the nursing team, helping us recognise concerns early and respond before they become bigger problems.

As needs change, we review the support being provided and work with the individual, their family and relevant professionals.

Everyone is different. Some people need a little support to stay independent, while others benefit from regular visits and ongoing oversight from our nurse-led team.


Support can include:

• Maintaining routines and independence
• Support with paperwork and everyday organisation
• Social connection and reducing isolation

• Liaising with families and other professionals when appropriate

Our aim is always to do with, not do for, supporting people to maintain confidence, choice and independence.

A Pathway That Can Change With You

Community support doesn't have to happen only at home.

Our Golden Community Hub at Little Orchard in Catworth is open for adults aged 18+, providing day activities, groups, social connection and health and wellbeing opportunities.

People can use the Hub alongside their community support, helping them build confidence, meet others and remain connected.

Our health and wellbeing drop-in clinics also provide opportunities to speak with our team and access appropriate advice, support and signposting.

We are developing Golden Community around continuity rather than fragmented services.

Home & Community → Golden Community Hub & Day Service → Little Orchard Six-Bed Nurse-Led Home

Our six-bed nurse-led home is currently in development and is not yet open.

The aim of this pathway is not to move people unnecessarily into higher levels of care. It is to support independence for as long as possible while providing familiarity and continuity if needs change in the future.
